Until January 23, 2026<\/strong><br>Chart requests can be sent by fax to 438-804-4618 <span style=\"text-decoration: underline;\">up to and including Friday, 23<br>January 2026<\/span>. Please sign and date your request, and specify whether the chart should<br>be faxed to another physician clinic or placed in the patient web portal as a pdf for your<br>personal use. <span style=\"text-decoration: underline;\">These requests will be handled promptly and at no charge to the patient<\/span>.<\/p>\n\n\n\n<p class=\"has-medium-font-size\"><br><strong>B. January 24, 2026 onwards<\/strong><br>From January 24, 2026 onwards, chart requests must be directed to the Coll\u00e8ge des<br>M\u00e9decins du Qu\u00e9bec.
