Chart requests
Please note that for patients returning to the care of a family physician or awaiting
transfer to a new psychiatrist, pertinent chart information has already been sent (family
physicians, some transfers to psychiatrists) or will be sent by Friday, January 16 th 2026
(other transfers to psychiatrists).
A. Until January 23, 2026
Chart requests can be sent by fax to 438-804-4618 up to and including Friday, 23
January 2026. Please sign and date your request, and specify whether the chart should
be faxed to another physician clinic or placed in the patient web portal as a pdf for your
personal use. These requests will be handled promptly and at no charge to the patient.
B. January 24, 2026 onwards
From January 24, 2026 onwards, chart requests must be directed to the Collège des
Médecins du Québec. Instructions can be found on the CMQ website.
