It is with a heavy heart that I inform you that I will be closing my private practice. I will be leaving the province and unable to take my current patients with me to my new workplace.
For current patients, discharge planning will be addressed at your next appointment if not already discussed. Please note that it is now possible to use the patient web portal to schedule or reschedule your clinic visit.
